View Dentist Directory

To view the Dentist Directory, select the Dentist Directory menu option from the left side navigation bar. The Dentist Directory screen appears. To select a directory, do the following:

The directories are arranged by state and by product. Select the directory that you want to print by clicking the product link for the correct state. The directory appears in a new window.
To print, select the print icon in toolbar. A print dialog box appears. When you have selected the correct settings for your document, select OK to print the document.

If you need a state not listed, fill out the Contact Form and the directory will be mailed within two business days.

Contact Form

To get a directory not specified on the Dentist Directory screen, provide the following information in the Contact Form section of the screen:

name — your name
email — email address to send the directory to (the default email address from the profile is automatically populated here)
state — select the directory state
product — select the directory product (Delta Dental Premier, Delta Dental PPO, or DeltaCare)

Select Submit to process your request.
