Claim Forms

If you need to submit a paper claim, here's how to do it:

Select Claim Forms from the menu in the left side panel. The Claims Forms screen appears, containing a link for the plan in which you are enrolled.

Select the link to open the Dental Claim Statement PDF. Print, fill out, and mail the form. Instructions and mailing addresses are included on the form itself.
